New Client Forms

Please print formsy you’ll need, fill them out and bring them with you for our first appointment. If you’re a couple, you’ll each need your own form. If we’re meeting through video/telehealth, you can scan the forms and email them back to me. Please send them to me at least 24 hours before our appointment, so I have time to read them. Thank you.
